Key Responsibilities

?? Coordinate timely referrals for vital services, including housing assistance, vocational rehabilitation, specialized mental health care, medical referrals, and substance use treatment.
?? Employ motivational interviewing techniques to educate and engage patients while managing follow-up care.
?? Maintain and regularly update the patient registry.
?? Accurately document care plans and clinical notes.
?? Actively contribute to interdisciplinary care team meetings to ensure continuity of care and address patient needs.

Qualifications

Compensation And Benefits
