Together, we can make a difference, one visit at a time.We are seeking compassionate Social Workers with excellent customer service skills to assist our Care Teams with advance care planning, cognitive assessments, placement, general resources, etc.
This is a salary-based position, with the opportunity to receive a monthly bonus. This is a full-time 8-hr remote position (8:00AM-5:00PM) (Monday-Friday).
The Social Worker Would Be Responsible For The Following
 Initiating Advance Care Planning for patients throughout their assigned Care Team (including facilities).
 Completing Daily Comprehensive Assessments on patients.
 Completing Daily Care Plans on patients.
 Documenting Chronic Care Management time daily.
 Meeting Daily Productivity Goals
 Coordinating discharges at contracted facilities
 Coordinating Placement.
 Continuation of coverage (i.e. coordinating acute visits for falls, med changes, etc.)
 Build and maintain professional relationships with referral sources.
 Maintain strong communication and professionalism between referral sources, patients/families, and Care Teams.
