Job OverviewWe are actively seeking a dedicated and empathetic Patient Care Manager to lead our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) initiatives. In this pivotal role, you will empower patients to navigate their chronic conditions effectively while ensuring the provision of exceptional care. Your efforts will be instrumental in creating personalized care strategies that enhance patient well-being and support their health journey.
Key Responsibilities
 Conduct comprehensive assessments to evaluate the medical, psychological, and social requirements of patients.
 Collaborate with a multidisciplinary teamÂincluding physicians, pharmacists, and other healthcare professionalsÂto formulate and execute tailored care plans.
 Deliver ongoing education on chronic disease management, encompassing medication adherence, vital sign monitoring, nutrition, exercise, and lifestyle changes.
 Facilitate effective communication among patients, caregivers, and healthcare team members to ensure coordinated care.
 Monitor RPM data to detect trends and potential health risks, sharing insights with relevant healthcare professionals.
 Maintain accurate documentation of patient interactions and care activities in the electronic health record (EHR) system.
 Assist with the enrollment of eligible patients into the CCM and RPM programs while assessing program effectiveness and patient satisfaction.
Required Skills
 Proficiency in electronic health records (EHR) systems.
 Familiarity with Google Workspace.
 Strong interpersonal and communication skills for effective patient education and teamwork.
 Excellent organizational skills and attention to detail, with the ability to manage multiple tasks simultaneously.
Qualifications
 Certified Medical Assistant (MA) or Licensed Practical Nurse (LPN) is required.
 At least 2 years of experience in patient care, care coordination, or chronic disease management. Experience with CCM or RPM programs is advantageous but not mandatory.
