About the position

The HSS Coordinator role at UnitedHealthcare is a vital part of the Integrated Care Model, focusing on advocating for members' needs and ensuring they receive appropriate care services. This position involves assessing, planning, and implementing individualized care strategies while managing care plans and communicating with stakeholders to provide quality coordinated care. The role is field-based, allowing for a hybrid work schedule for those residing within a specified distance from San Antonio, TX.

Responsibilities
• Assess, plan, and implement individualized care strategies for patients.
,
• Identify and initiate referrals for social service programs, including financial and community support services.
,
• Manage the care plan throughout the continuum of care as a single point of contact.
,
• Communicate health-related information to stakeholders to ensure quality care is provided.
,
• Advocate for patients and families to ensure their needs and choices are represented.
,
• Visit members in their homes and other settings such as community centers and hospitals.

Requirements
• High School Diploma/GED (or higher) AND 5+ years of experience working with ABD/SSI population within a clinical health care role OR Associate's Degree with 3+ years of clinical experience OR Unrestricted LPN/LVN license in the state of TX with 3 years of clinical experience OR CHW certification with 3+ years of clinical experience.

• Familiarity with Microsoft Office, including Word, Excel, and Outlook.

• Reliable transportation and ability to travel in the assigned area to visit Medicaid members.
,
• High-speed internet in place of residence.

Nice-to-haves
• Ability to create, edit, save and send documents, spreadsheets, and emails.

• Reside within commutable distance of assigned duties.
